UBTH Community Health Officers Admission Form 2021/2022

University of Benin Teaching Hospital (UBTH) Center for Training Community Health Officers (CTCHO), Ekpoma Higher National Diploma (HND) Admission Form, Requirements, Application Guidelines, Deadline, Interview Dates, Time and Venue for 2021/2022 Academic Session.

Applications are hereby invited from suitably qualified candidates for admission into the Community Health Officers Training Programme for 2021/2022 Academic Session.

The training, which lasts for two (2) calendar years will commence in September, 2021. It leads to the award of Higher Diploma in Community Health.


Admission Requirements:

Basic educational equalization: Candidates must possess five Credit passes obtained in not more than two sittings in WASCE/GCE/NECO) in the following subjects, English Language, Mathematics, Biology, Physics and Chemistry. Additional qualifications: Candidates must possess any of the following professional qualifications and experience:

i. Diploma in Community Health (DCH) with not less than two (2) years post qualification experience.
ii. Community Health Extension Worker (CHEW) with five (5) years post qualification experience.

Method of Application:

Admission forms are obtainable at the office of the Coordinator, Centre for Training Community Health Officers UBTH, Benin City, on payment of a non-refundable fee of Ten Thousand Naira (N 10, 000) only into UBTH Account with T SA using REMITA platform in any Commercial bank in Nigeria. Deadline for obtaining and submission of completed admission form is July 2nd, 2021.

  • Venue: Institute of Health Technology, UBTH, Benin City.

Candidates are to bring originals and photocopies of the following to the examination Centre.
  • Certificate / Master-list
  • Bank Receipt (evidence) of payment of application fee.
  • Valid or Current Practice License.
  • SSCE/GCE/NECO
  • CHEW Certificate/Abridged Certificate DCH Certificate (as applicable)
  • Evidence of change of name (if applicable)
