SON-UCH 2022/2023 Admission Form University College Hospital (UCH), Ibadan School of Nursing Admission Form, Admission Requirements, Application Guidelines, Entrance Examination Date and Closing Date for 2022/2023 Academic Session.
Applications are invited from suitably qualified candidates for admission into the School of Nursing, U.C.H., Ibadan for October 2022 Class.
The course is for a period of three years, following which candidates shall be presented for both the School and Nursing & Midwifery Council of Nigeria Final Examinations to qualify, be registered and licensed to practice as General Nurses (RN)
The General Nursing Entrance Examination Committee of the Hospital will conduct the Entrance Examination.
Only candidates who attained acceptable standard in the Examination will be considered for admission after passing the Interview exercise to follow the Examination.
Admission Requirements:
Only interested candidates who should not be younger than 17 ½ years of age and possess the West African Senior Secondary School Certificate (WAEC) or NECO with Credit passes in at least five (5) subjects which must include English Language, Mathematics, Biology, Chemistry, Physics at not more than two sittings may apply.Method of Application:
Candidates should request for Application Forms with effect from Wednesday, 3rd August, 2022 at the through the UCH SCHOOLS’ PORTAL at website https://portal.uch-ibadan.org.ng. Interested individuals are to do the following;(i) Make a payment of a non-refundable fee of Seven Thousand, Five Hundred Naira (N7,500.00)
only through any commercial bank into University College Hospital, Ibadan T.S.A. Account- School of Nursing. Teller to be changed to receipt at UCH Accounts Department.
(ii) Make a payment of Two Hundred Naira (#200.00) only into Oyo State Nursing & Midwifery Committee Account, First Bank Plc. 2019704292.
(iii) Present both the UCH receipt and First Bank teller for the payment of #200.00 at Room 6, School of Nursing to collect PIN (Personal Identification Number) for login and online application submission.
(iv) Go to the portal on https://portal.uch-ibadan.org.ng, download the Applicants’ User Guide
and enter your PIN (Personal Identification Number) to login and create your account.
(v) Follow instructions on the Applicants’ User Guide to input your data and submit your application online.
(vi) Print 2 copies of your form and Photo card and submit for authentication at Room 6, School of Nursing
Registration Guidelines:

1. Open a browser, preferable Google Chrome
2. Type URL: https://portal.uch-ibadan.org.ng
3. You’ll be redirected to the landing page
4. Click Enter Pin button
5. Enter Pin obtained from the school and Click Login Button as shown below and you will be taken to where you will create your credentials.
6. CREATE ACCOUNT FORM: Fill in your credentials as stated below and click Create Button and you will be taken to home page to login with your credentials
7. LOGIN: Click the login and login page bellow will be popped up, Click login Button
Click Forget Password/Change Password link: In case you forget your password and need to change it. Please note that any form that’s not filled is marked red and once filled it is marked white.
8. BIODATA FORM: Fill in all required fields and and Click Save Button. If the form is submitted, update can also be made by changing any field and click the Update Button
9. SCHOOL: Click on school and a form is popped up, select your school and Click Save. If the form is submitted, update can also be made by changing any field and click the Update Button5
10. PASSPORT PHOTO: Click on Passport Photo Button and a form is opened up, upload your
passport (White Background not more than 300kb) and Click Save. You can also change your passport.
11. PERMANENT ADDRESS: Fill in, all the fields are required and Click Save. If the form is submitted, update can also be made by changing any field and click the Update Button
12. CONTACT ADDRESS: Fill in, all the fields are required and Click Save. If the form is submitted, update can also be made by changing any field and click the Update Button
13. O’ LEVEL QUALIFICATION: Fill in the required fields and Click Save. To update and delete, click the Update/Delete Button
14. UPDATE O’ LEVEL QUALIFICATION: Click Edit button to update and Delete button to
remove any record. In case you want to update, fill in or select any field you want to update and click the Update Button.
15. ACADEMIC RECORDS: Fill in, all the fields are required and Click Save. To update and delete, click the Update/Delete Button
16. UPDATE O’ LEVEL QUALIFICATION: Click Edit button to update and Delete button to remove any record. In case you want to update, fill in or select any field you want to update and click the Update Button.
17. SPONSOR: Fill in, all the fields are required and Click Save. If the form is submitted, update can also be made by changing any field and click the Update Button.
18. NEXT OF KIN: Fill in, all the fields are required and Click Save. If the form is submitted, update can also be made by changing any field and click the Update Button.
19. REFEREE: Fill in, all the fields are required and Click Save. To update and delete, click the Update/Delete Button
20. UPDATE REFEREE: Click Edit button to update and Delete button to remove any record. In case you want to update, fill in or select any field you want to update and click the Update Button.
21. PREVIEW Form: Click Preview Form Button to preview the form before printing.
22. PRINT FORM: Click Print Button, scroll down and click Print Button to print the form.
23. PHOTO CARD: Click Photo Card Button, click Print Button to print the Photo Card on Thick Paper (Please Print two (2) copies).
NB
1. Print the Form, attach all relevant documents, photocopies of all credentials and submit a copy to the School.
2. Print two (2) Photo Cards and take it to the school for Stamping and Signature
Method of Selection:
UCH IBADAN ENTRANCE EXAMINATION WILL BE ANNOUNCED LATER.
(i) Applicants
shall be notified of the date, venue and time for Computer Based (CBT)
Entrance Examination through their e-mail addresses and text messages
(ii) Only candidates with authenticated (stamped and signed) Photo cards shall be allowed into examination venue.
(iii)
Only candidates who attain acceptable standard at the examination shall
be invited for interview or considered for selection following
interview.

Is the examination result of July 12th not out yet?
