LAUTECH Authentication of Medical Report Guidelines

Ladoke Akintola University of Technology (LAUTECH) Publishes Current Guidelines for Authentication of Medical Report for all Newly Admitted and Returning Students of the Institution.

In recent time, the University Health Centre has been bombarded with catalogue of medical reports from other Hospitals  in respect of staff and students requesting for confirmation of authenticity of such documents for the purpose of covering for period of suspension of academic activities or absence from duty post on Health ground. It is therefore imperative to remind the University community of guidelines governing such requests.

Students and staff under hospital admissions outside the University health centre are adviced to notify the University by sending an interim medical report from such facility to the office of Director of Medical Services pending final discharge.

Staff and students treated or hospitalized in any hospital facility outside the University Health Centre, thereby missing academic/ duty engagement are required to apply to the Director of Medical services for authentication of medical reports/ documents brought from such hospital within three (3) months of discharge from the health facility.

Any request for suspension of academic activities, absence from duty post or request for financial assistance from university management on Health ground brought into Departments/Faculties  must be rooted through the office of the Director of Medical Services before being processed to the Registrar.

Spurious medical reports from private clinics/ hospitals WILL NOT be considered.

All Head of Departments and Dean of faculties are to kindly note that Confirmation of authenticity of any  Medical reports brought from outside the University is the prerogative of the office of Director of Medical Services.

Thanks for due compliance
